Practices Benefit from Industry-Leading Care Management Services and OCM Designation

FORT MYERS, FL (MARCH 19, 2019) – Through the American Oncology Network (AON), LLC, community oncology practices across the nation are now able to leverage the expertise and scale developed by the nation’s largest independent community oncology practice to actualize value-based care without managing the growing complexities of a changing healthcare landscape. New AON practices are immediately eligible to participate in the Oncology Care Model (OCM) and implement the AON Care Management program. In 2018, AON adopted OCM and implemented the model through Care Management, its existing value-based approach. This growing team-based methodology ensures clear communication, coordination and cohesion of patient care. It also optimizes treatment outcomes and quality of life through a three-phase program that includes care navigation, active care management and survivorship.
