Care Coordination

Ongoing care coordination is provided by experienced oncology clinicians and support staff who are available to assist you throughout your treatment with symptom management and nutrition needs.

The goal of care coordination is to help you manage your physical, psychosocial and emotional needs as well as act as your primary contact. The care coordination team performs many functions including providing medication and diagnoses education, symptom monitoring, as well as addressing barriers to care, including financial and transportation assistance. Your care coordinator will assist you with creating and understanding your personalized plan of care and help facilitate communication between your oncologist and other providers.

Patients enrolled in programs such as Principal Care Management (PCM) or Enhancing Oncology Model (EOM) program will be assigned a care coordinator. The PCM program is available to Medicare and most Medicare Advantage patients who have one serious, complex chronic condition. EOM is specifically designed for Medicare Fee-For-Service beneficiaries with seven specific types of cancers: breast cancer, chronic leukemia, small intestine/colorectal cancer, lung cancer, lymphoma, multiple myeloma, and prostate cancer.

Patients also have access to a wide range of services and resources that help improve their overall health and well-being including support from dietitians to minimize the impact of cancer on daily life.

Our approach to cancer care is always patient focused and individualized, whether care is delivered in a personal visit at an office or via a video chat. Telehealth services, when appropriate, provide an excellent option for patients who cannot travel, or wish to reduce travel, to their doctor’s office.

  • Receive personalized care in the comfort of your home, or virtually anywhere
  • Reduce the time, cost and effort associated with traveling to an in-person office visit

  • No need to download software or create an account, simply use a computer or mobile device
  • Just like a regular appointment, your virtual visit is entirely private and confidential
DigniCap AON

Hair loss is a well-documented side effect of many chemotherapy drugs. The DigniCap® Scalp Cooling System offers you the possibility to minimize hair loss from certain chemotherapy drugs. For over a decade DigniCap has successfully helped thousands of patients globally. At select locations, we offer the FDA-cleared DigniCap treatment as an option for our patients to help diminish the burden from hair loss.

DigniCap features a patented cooling cap that is placed directly on the head. A liquid coolant circulates throughout the cap, delivering consistent and controlled cooling to all areas of the scalp. These factors together reduce the risk of hair loss.

DigniCap can be used by patients receiving chemotherapy to treat solid tumors for breast cancer as well as prostate, ovarian, uterine, lung and other tissues. Please note that scalp cooling is effective with many chemotherapy drugs, but not all. Your doctor can tell you if scalp cooling is compatible with your treatment.
