Form 1095-C Availability Notice

Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is a federal tax document required under the Affordable Care Act. This form provides information about health plan coverage that may have been offered to you by your employer during the previous calendar year.

A 1095-C form may be used to:
• Confirm whether you were offered health plan coverage by your employer.
• Assist in preparing your federal income tax return.
• Respond to inquiries from the Internal Revenue Service, if applicable.

How to Request Your Form 1095-C

For current and former employees of American Oncology Network, copies of 1095-C forms are available upon request.
To request a copy of your form, please email AONBenefits@aoncology.com.
Requested forms will be provided within 30 days of receipt. Please note that if you were not a full-time employee during the requested calendar year, a 1095-C form may not be available.
